Medication Renewal:

A: Book renewal appointment  2-3 weeks before you run out of pills or 4-6 weeks before you run out of blister packs See our medication renewal tips 

Appointments:​

A: Book regular appointments online for in-person or telephone  consultations using your PHN and your doctor’s name. You cannot book with another doctor or nurse in the clinic. Call the office for same-day appointments if not available online; they will need to be triaged by the front desk staff

A: Cancellations with your Crossroads doctor have to be made more than 24 hours before the
appointment time. Late cancellations are subject to a fee of $50. Please call the specialist’s
office if you cannot make your appointments there – they are subject to cancellation fees as
well. Imaging appointments you have to cancel or change are to be done directly with the
hospital imaging facility

A: Call and book an appointment , with the type of form that needs filling. Fill out the patient
portion, sign and date the form before the appointment. You can email or drop this off prior to
your appointment. Your doctor will complete this form, usually in your presence. You will be
invoiced on completion. Payment is accepted in cash, cheque or debit/credit card. eTransfers
are not possible.

Sick note:

A: Yes we can see you if your symptoms require a visit. You may request a sick note at that time. If you are well and only need a sick note, please use this  Generic Sick Note  issued by the BC College of Family Physicians. Sick notes generate a service fee not covered by the medical services plan.

Waiting for Test Results:

A: Test results are available online through lab services such as LifeLabs and Fraser Health Labs if you pre register. Patients should ensure their contact information (Cell # or Email) is updated to receive notifications of results that require follow-up.

A:You will be notified of results that need to be followed up by text, email or phone call as per
your preference. Please ensure your contact details are up to date at all times These messages
will not have detailed medical information for privacy reasons.
Routinely, patients are not notified of normal results unless requested by prior arrangement with
the ordering physician. Front desk staff are not able to provide results to patients. Make a virtual
or clinic appointment to discuss your normal test results with your doctor if necessary.

Waiting for Imaging Appointments:

A: Contact the office after the time interval discussed with your doctor at the time of imaging request. IF no time was discussed, wait at least 1 months before checking back.

Waiting for Specialist Appointments:

A: Timelines vary depending on the specialist and urgency. Please check with the doctor or front desk for a time to check back before you leave the office.
